What is Shared Decision Making?




  • A process through which clinicians and patients share information with each other and work toward decisions about treatment chosen from medically reasonable options, that are aligned with the patient’s values, goals and preferences.
  • Patients living with HF need to know the trajectory of illness and the important decisions they will face.
  • Decision support helps patients think through, forecast, and deliberate their options.
  • Assumes family, caregivers, companions are involved.
  • Some may not be interested in the shared model
