After Conversation

    Collaborate with patient/family to plan future care
  • Tailor therapy based on preferences and goals
  • Refer to other services that address their most important challenges (Psych, PT/OT, Palliative)
  • Change treatments to maximize the chance they will get what they want
  • Provide resources they can investigate on their own-written, internet, pamphlets
  • Introduce the idea of completing advanced directives to document their decisions
