Elements of Prognosis



    After gaining information on what they want to know and their goals:
  • Discuss what you expect their trajectory to be
  • Discuss important decisions they will be asked to make
  • Discuss the options of HF care, risks/benefits, Side Effects, and the impact they can expect
  • Offer your opinion if they want it
  • “If this was me, I would likely choose….”
  • “If this was my parent, I would want XXX for them”.
