The Ascertain Dementia 8-item Informant Questionnaire (AD-8) is an 8-question screening tool that reliably detects early cognitive changes based on intra-individual decline. It has been validated in its ability to distinguish the onset of mild dementia from a normal baseline. The AD-8 form is best completed by the patient’s family member or caregiver.
