Mr. Reynolds is a recently (12 months ago) widowed 85-year-old white male with heart failure (HF). He now lives with his daughter and her family, and they all get along well. The last time his ejection fraction was measured 3 months ago, it was 35%.
Comorbidities:
Hypertension, coronary artery disease (for which he underwent triple-vessel coronary artery bypass grafting), osteoarthritis, hyperlipidemia, and asthma.
Current Medications:
- Lisinopril (Zestril) for blood pressure and HF management
- Carvedilol (Coreg) for blood pressure and HF management
- Aspirin, Clopidogrel (Plavix), blood thinners
- Simvastatin (Zocor) for hyperlipidemia management
- Furosemide (Lasix) for edema management in HF
- Albuterol inhaler (Proventil) for asthma management
- Clonazepam (Klonopin) for insomnia and anxiety management
- Over-the-counter Naproxen for back pain and Diphenhydramine for insomnia
