Next Steps: Cognitive Impairment in HF

    If cognitive impairment is significant (positive Mini-Cog AND OR AD8 scores):
  • Refer the patient to a provider or care center that specializes in neurological, mental health, and memory disorders
    • Also refer for evaluation if depression is suspected
  • Educate family caregivers about the need of involvement with treatment regimen management
  • Control HF
  • Routinely re-assess CI
  • Review medications and reconcile medication list
    • Minimize the use of inappropriate medications (including herbal and complementary products)
    • Use lowest dose that is effective, avoid polypharmacy when possible
