- Identify possible causes of heart failure and/or acute exacerbations of heart failure.
- Recognize signs and symptoms of acute exacerbation of heart failure.
- Understand the assessment and management of a patient with an acute exacerbation of heart failure, specifically integrating guideline-directed medical therapy (GDMT) in a patient-centered manner.
- Identify the role of each member of the multidisciplinary team in the management of patients with heart failure.
- Recognize important factors in transitions of care for heart failure patients that reduce morbidity and mortality and increase quality of life.
