Case objectives:

    By the end of this case study, the learner should be better able to:
  1. Identify possible causes of heart failure and/or acute exacerbations of heart failure.
  2. Recognize signs and symptoms of acute exacerbation of heart failure.
  3. Understand the assessment and management of a patient with an acute exacerbation of heart failure, specifically integrating guideline-directed medical therapy (GDMT) in a patient-centered manner.
  4. Identify the role of each member of the multidisciplinary team in the management of patients with heart failure.
  5. Recognize important factors in transitions of care for heart failure patients that reduce morbidity and mortality and increase quality of life.
