Initial depression screening

A provider conducted a routine assessment as recommended by the American Heart Association. The initial screening revealed the following symptoms:

  • Difficulty sleeping
  • Daytime fatigue
  • Feeling sad, teary
  • Feeling hopeless
  • Lack of motivation
  • Passive suicidal thoughts

Patient scored 17 on Patient Health Questionnaire (PHQ-9).
