Mr. Andy Johnson is a 26-year-old Caucasian male status post heart transplant 10 years ago for hypertrophic cardiomyopathy. He has a history of depression post-transplant and one admission after stopping his medications (including his anti-rejection medications). This time, he reported not taking his medications for about 2 months because he was too tired to refill them. He has felt well until about 2 weeks prior to his admission to this cardiac inpatient unit, when he began to notice irregular heartbeats, and developed chest pain. He denied any shortness of breath, and his exercise tolerance was unchanged.
